Continuity Care have grown rapidly and are looking for a full-time Community Nurse/Care Coordinator to support our growth in Brisbane, you may also be required to travel to Gold Coast or other regions of QLD as per business needs. The ideal candidate will need to have at least 2 years of work experience in a similar position and believe in our vision of keeping people connected to and engaged with friends, family, homes and communities they love for as long as they desire is the only way to maintain Quality of life. Primary Objective:To provide clinical governance along with effective and efficient nursing care to the people with disability and aged in community maintaining holistic care using Continuity Care Model under NDIS/CHSP/HCP/DVA/Private.The Care Coordinator (Clinical) role will:Provide clinical leadership and drive services and business development opportunities for care and support services in partnership with the operations manager with in social and community health settings.You will be passionate about supporting people to live a full life in their homes, undertaking a goal-orientated approach to developing and updating clinical and nonclinical comprehensive support plans with clients.You will provide support with clinical supervision, assessment & recommendations for clients accessing our services.Act as an advocate to the Support and Services team to identify and articulate medical needs and interventions where requiredWork with the Operations Manager to ensure that organisation meets the requirement for NDIS/My aged care accreditation and professional practise standardsLead clinical governance and manage the quality management framework within continuity care and its services to ensure care standards are metImplement and maintain regular meeting schedules, including daily management system huddles, monthly team meetings with KPI reporting, performance reviews and supervision with direct reports.Monitor the work practices of all care staff and provide direction, guidance and training as required to ensure the provision of safe and quality care which meets Aged Care Quality and Safety Standards.Participate in, contribute to and implement quality improvement and risk management in all aspects of service.Provide direct and indirect clinical care to care recipients within scope of practice and ensure clinical needs are maintainedProvide advanced holistic care skills within a Case Management framework working towards Continuity of Care modelContribute to the ongoing development of the program including participating in development of procedures, planning, and reviewing of care recipient servicesMeet on a regular basis with the Operations Manager to provide reports on achievement of individual and organisational KPIs.Provides feedback to Operations Manager to identify staff needs and positive achievements and further training development/delivery and monitorsupervising and coordinating the work of support and health care workers within organisations.You will also have the opportunity to maintain oversight over clients' outcomes and changes in circumstances during their time with the organisation supporting them to lead more independent lives. What we need from you:Mandatory Nursing bachelor�s degree qualification and APHRA registration as well as comparable experience in HCP/CHSP/DVA/NDIS.Apply advanced clinical nursing/midwifery expertise, including assessment, planning, implementation and evaluation, to deliver a high standard of consumer/person centred care.Demonstrated work experience with service delivery team on Restrictive Practise and human right framework to ensure that organisation meets it reporting obligation to NDIS and My Aged Care.
