What is a care assessment?+

An independent assessment considers strengths, daily living needs, safety, goals and the supports that may be appropriate. The assessment organisation—not Continuity Care—determines eligibility and approvals.

What is Support at Home?+

Support at Home is the Australian Government program for coordinated aged-care services that help eligible older people remain at home.

What is Self-Management?+

Self-Management gives you more involvement in choosing providers, schedules and everyday care decisions, with guidance available when needed.

What is Full Management?+

Full Management means our care team coordinates agreed services, schedules, providers, administration and reviews with you.

What is care coordination?+

Care coordination brings different services, providers, schedules and communication together around one care plan.

What is care navigation?+

Care navigation helps you understand assessment, funding, provider and management options so you can make informed choices.

Can my family be involved?+

Yes, where you authorise it. You decide who is involved and what information can be shared.

How are services paid for?+

Funding and contributions depend on assessment outcomes, services, individual circumstances and current government rules. Agreed prices are documented before services begin.

What happens if my needs change?+

Contact the care team for a review. Service arrangements may be adjusted and a reassessment may be appropriate.

Can I change management style?+
