What is Support at Home?+
Support at Home is the Australian Government program for coordinated aged-care services that help eligible older people remain at home.
What is Self-Management?+
Self-Management gives you more involvement in choosing providers, schedules and everyday care decisions, with guidance available when needed.
What is Full Management?+
Full Management means our care team coordinates agreed services, schedules, providers, administration and reviews with you.
What is care coordination?+
Care coordination brings different services, providers, schedules and communication together around one care plan.

Can my family be involved?+
