Our approach
Care that begins with what matters to you.
Good in-home care should protect your independence—not take it away. We listen to your routines, preferences, culture, relationships and goals, then help coordinate an agreed mix of services around them.
Support may be available through Support at Home, an NDIS plan, another funded pathway or a private arrangement. Eligibility, service availability and costs are confirmed before support begins.

Funding and pricing
Understand funding and costs before care begins.
The pathway depends on your assessment, approved supports and individual circumstances. Continuity Care confirms the current service price, inclusions and any contribution or gap before you agree.
Request current pricingSupport at Home
Services must align with your approved support plan and current program rules. Participant contributions vary by service category and financial assessment.
NDIS
Supports must relate to the participant’s disability needs, goals and available plan funding.
Private arrangements
Where available, privately funded services are documented in a written agreement before delivery.
Important: Funding decisions are made by the relevant government agency. Service availability is not guaranteed until Continuity Care confirms scope, capacity, funding and a written agreement.
Common questions
Clear answers before you decide.
Contact our team if your situation is not covered here.
Who can receive in-home care?
Availability depends on the person’s needs, location, funding pathway, service suitability and current workforce capacity. An initial conversation helps us identify the appropriate next step.
Can I choose which services I receive?
Yes. Agreed services are shaped by your goals and preferences, within the scope of approved funding, safety requirements and availability.
Can family or carers be involved?
Yes, when the person receiving support authorises their involvement and information sharing.
What happens when my needs change?
Contact the care team. We can review current arrangements, coordinate changes within scope and discuss whether reassessment is needed.
Start with a conversation
Let’s make the next step clearer.
Tell us what matters, what funding or referral you have and where you need support. We will respond honestly about scope, availability and next steps.
