Our approach
Professional nursing with a whole-person view.
Community nursing begins with an appropriate clinical assessment and a clear plan. We work within professional scope, document care, communicate agreed changes and coordinate with treating practitioners and other supports where consent is provided.
Specific nursing supports depend on clinical need, practitioner scope, funding, location and workforce availability. Urgent or emergency care is not provided through this service.
What support may include
Nursing support matched to assessed needs.
Each service is individually agreed and subject to assessment, funding, suitability and current availability.
Nursing assessment and care planning
Review health-related support needs, risks, goals and required clinical instructions.
Medication support
Clinical oversight or support within an authorised medication plan and practitioner scope.
Wound and skin integrity support
Assessment, monitoring and care delivered under an appropriate clinical plan.
Continence and catheter support
Individual support, education and monitoring where clinically appropriate.
Diabetes and complex health support
Disability-related or aged-care nursing support according to current instructions and funding.
Education and coordination
Help the person and authorised supporters understand the care plan and when to escalate concerns.
Focused on outcomes
Safer care through observation, communication and review.
The nurse considers the person’s health needs in the context of home, routines, communication preferences and other services. Care is escalated to the appropriate health professional when it sits outside community nursing scope.

Funding and pricing
Clinical funding depends on the care pathway.
Community nursing may be funded when it is included in an approved aged-care support plan or is an eligible disability-related health support in an NDIS plan. Private arrangements may also be considered where available.

Eligible clinical supports such as nursing are government-funded under current program contribution rules. Service scope and price are agreed before delivery.
NDIS
The support must meet NDIS funding criteria and relate to the participant’s disability support needs—not treatment that is the responsibility of the health system.
Private or other funding
We confirm clinical suitability, price and payment responsibility in writing before services begin.
Important: Funding decisions are made by the relevant government agency. Service availability is not guaranteed until Continuity Care confirms scope, capacity, funding and a written agreement.

Is community nursing an emergency service?
No. Call Triple Zero (000) for an emergency. For urgent medical advice, contact an appropriate medical service.
Do I need a referral or clinical instructions?
This depends on the requested support. We will explain what information, referral or current clinical plan is required before accepting the service.
Can a nurse train support workers or family?
Where clinically appropriate, funded and within scope, a qualified practitioner may provide person-specific education or competency support.
Will you communicate with my GP or specialist?
Yes, with appropriate consent and where communication is relevant to safe, coordinated care.
