The service we can provide depends on the person’s funding program, approved arrangements, current provider scope and service availability. We confirm this before commencement.

NDIS Plan Management
A registered plan manager helps administer the funded plan: processing compliant provider invoices, making payment claims, tracking budget use and providing agreed statements or information. The participant continues to choose providers and direct their supports.

Support at Home Care Management
Care management is part of ongoing Support at Home. A care partner works with the person on care planning, service planning, monitoring, review, support and education so services remain aligned with assessed needs and goals.

Other Coordination and Administration
Some people need private navigation, coordination or administration outside these funded roles. We first confirm the purpose, authority, scope, funding responsibility and price. This service is not presented as government-funded plan management.

Fees and claims must follow the current NDIS pricing arrangements and the participant’s funded plan-management supports.
Support at Home care management
For ongoing services, care management is funded through the program’s care-management allocation. Under current rules, participants do not contribute to care-management activities.
Other arrangements
Any private coordination or administration service must be separately scoped, priced and agreed; it is not represented as NDIS plan management or government-funded aged-care management.
Important: Funding decisions are made by the relevant government agency. Service availability is not guaranteed until Continuity Care confirms scope, capacity, funding and a written agreement.

Is NDIS plan management the same as support coordination?
No. Plan management focuses on funding administration and provider payments. Support coordination focuses on implementing supports and building participant capacity.
Is aged-care care management the same as NDIS plan management?
No. Support at Home care management includes care planning, service planning, monitoring, review, support and education. It follows aged-care program rules rather than NDIS plan-management rules.
Can plan management change what the NDIS funds?
No. A plan manager administers funding in line with the plan and current rules. The NDIA makes funding decisions.
How will I know the price?
We confirm the applicable current price, funding arrangement and service agreement before commencement. Ask our team for the current price schedule relevant to your pathway.
