Refer a Client

An NDIS plan reassessment conversation is an opportunity to discuss your circumstances, goals and current supports. Preparing early can make it easier to provide clear, relevant information.

Review your current plan

Look at each goal and support. Note what has helped, what has not been used and why. Keep examples of progress and barriers from everyday life rather than relying only on general statements.

Record changes

Consider changes to your health, living arrangement, informal support, work, study, transport or community participation. Gather recent assessments or reports that explain your functional needs and recommended supports. Documents should be current and connected to your goals.

Write down your priorities

Prepare a short list of the outcomes you want to discuss. You may also want a family member, nominee, advocate or support coordinator to attend. Confirm the meeting format and request accessibility or communication adjustments in advance.

After the conversation

Keep copies of information supplied and note any next steps. When you receive a new plan, review the funding periods, categories and stated supports carefully.
