Refer a Client

Returning home after hospital can be a relief, but the first few days may involve new medications, appointments, equipment or changes to daily routines. Planning should begin before discharge whenever possible.

Ask for written information

Confirm medication instructions, follow-up appointments, warning signs and who to contact with questions. Make sure discharge information is provided in an accessible format and that the person going home understands the plan.

Check the home environment

Consider safe access, mobility, bathroom arrangements, meal preparation and where essential items will be kept. Equipment should be delivered and set up before it is needed, with instructions for safe use.

Coordinate the people involved

With the person’s consent, identify who will communicate with the hospital, GP, allied health professionals, nursing service, support workers and family. Clear responsibilities reduce duplication and missed tasks.

Plan for changes

Know what to do if symptoms worsen or support does not arrive. For a serious or life-threatening emergency in Australia, call Triple Zero (000).

Continuity Care can collaborate with participants, families and health teams to arrange appropriate community-based support following discharge. Clinical discharge decisions remain with the treating health team.
