Community nursing can bring qualified clinical support into a familiar environment. Depending on assessed needs and the agreed scope of care, this may include wound support, health monitoring, continence-related care, medication-related support or health education.
Before the appointment
Gather relevant discharge information, medication lists, care instructions and contact details for your treating team. Make a note of recent changes, symptoms or questions. Tell the provider about communication, accessibility or cultural needs before the visit.
During the visit
The nurse should explain the purpose of the visit, seek consent and discuss the assessment or care being provided. Share what you are comfortable managing yourself and where you or your support network need guidance.
Ask what signs to monitor, how information will be documented and who to contact if something changes. If ongoing visits are planned, clarify the expected schedule and how updates will be shared with relevant health professionals—with your consent.
After the visit
Keep written instructions somewhere accessible and note any follow-up appointments. For urgent or worsening health concerns, use the emergency or health service recommended by your treating clinician; community nursing is not a substitute for emergency care.
